Upfront payment packages for IVF / ICSI beginning at $10,000 are now available for treatments. We include a full IVF cycle package and also break down the IVF cycle, depending on your specific situation. (Please refer to the packages below.) Certain procedures are excluded and medications are not included in these packages.

UC IUI (Artificial Intrauterine Insemination) Cycle Packages A - B

Package A : Artificial Intrauterine Insemination (IUI) procedure

Purchased within Total IVF Cycle Package (G) and must be used within 2 years of most recent retrieval. If IVF Cycle Package (G) was purchased then each subsequent transfer of frozen embryos (generated from that cycle) will be charged separately. Package (G) includes the first frozen embryo transfer.

Package G : Total IVF Cycle (Includes Packages A through F1)

You may choose to use donated eggs, if it is determined that this is an alternative for you. If you choose to use a donor, or choose to use a surrogate, there is an additional administrative package that we offer to cover the extra medical care provided when donor oocytes, or a surrogate, is used. All costs associated with identifying, obtaining and screening the donor are the responsibility of the Patient and are not included in the program unless previous arrangements have been made.

Full payment is due at the following:

Package OD: prior to evaluation of the oocyte donor
Package OR: prior to initiation of treatment
Package GC: prior to initiation of treatment
Package SD: prior to cryopreservation of sperm

Payment may be made by cash, personal check, Mastercard or Visa.
All package prices are subject to change.
All services must be provided at the University of Chicago Center for Reproductive Medicine & Fertility.
Packages are all inclusive and cannot be modified.
Packages are for laboratory and administrative costs only. Medical care including physical exams and treatment are not covered under these packages.

Refunds are possible in the following situations:

If the oocyte donor or gestational carrier is determined to be ineligible at time of History & Physical, then the intended recipient will receive a full refund for purchased packages.

Package OD: If the oocyte donor is determined to be ineligible after Health and IVF labs have been processed, but prior to FDA labs, the intended recipient may apply the current oocyte donor package (Package OD) towards the next cycle with a new donor or be refunded $700. After starting stimulation medications, there are no refunds.

Package OR:
If the oocyte donor is determined to be ineligible then the intended recipient will receive a $350 refund.

Package GC:
If the gestational carrier is determined to be ineligible after Health and IVF labs have been processed, but prior to FDA labs, the intended recipient may apply the current gestational carrier package (Package GC) towards the next cycle or be refunded $2,000. After the mock embryo transfer, there are no refunds.

Package SD:
There are no refunds associated with this package.
